Healthy opportunities Pilot program
The Healthy Opportunities Pilot (HOP) Program is an innovative Medicaid-funded program aimed at reducing the risks of adverse health outcomes through early interventions addressing social determinants.
At the Foothills Food Hub and McDowell LFAC, we take pride in serving HOP participants by delivering FREE healthy food boxes directly to their doors. These boxes are filled with food purchased from local farmers and pantry staples.

Please complete the following form to request a HOP referral. Please keep in mind that all information you submit in this form will be viewed by the McDowell Local Food Advisory Council HOP Team members and shared with necessary parties to determine eligibility.

Impact Health is an Asheville-based nonprofit leading a network of more than 50 human service organizations participating in North Carolina’s Healthy Opportunities Pilot. HOP is a groundbreaking initiative to improve health that uses Medicaid funding to support social drivers of health across four key domains: food, housing, transportation and interpersonal safety and toxic stress.
